Once completed, the form includes details about the individual’s routines, habits, appearance and other people involved in their care. It should be accompanied by an up-to-date photo.
You keep the form at home but hand it in to the Police if your person goes missing.
If your person goes missing and you believe they’re at risk of harm, phone 999 as soon as possible and tell the operator you have a Herbert Protocol form.
The Police will return the form when the missing person is found.
